    報名表填寫 Completion of the Registration Form
    每位參加者只需填寫一份報名表。
    Each participant is required to complete only one application form.

    重複報名處理 Processing of Duplicate Registrations
    如同一參加者於同一天重複提交報名，將以最後一份報名紀錄為準。已確認及繳費之報名恕不設退款或更改。
    Duplicate registrations for the same date will be processed based on the latest submission. No refunds or changes will be made for confirmed and paid registrations.

    多日報名選擇 Multiple-Day Registration Options
    如欲報讀多於一天的營期，請於同一份報名表內選擇所需之日期。
    Participants registering for multiple days may select more than one day camp date within the same form.

    年齡限制 Age Limitation 
    本日營只接受年齡介乎 6 至 12 歲 的兒童報名。
    Only children aged between 6 and 12 years old are eligible to register for the day camp.

    證書頒發 Certificate Issuance
    所有成功完成營期的參加者，將獲頒發由 GO PARK Sports 簽發之活動完成證書。
    All participants who complete the day camp will receive a certificate awarded by GO PARK Sports.

    健康與安全聲明 Health and Safety Statement
    參加者及其家長/監護人須確認其身體狀況適合進行日營活動，並能在無醫療輔助或其他協助情況下完成活動。GO PARK Sports 有權取消不適合參與活動者之報名。
    Participants and their parents/guardians must ensure the participant is in suitable physical condition to take part in the camp without medical or additional assistance. GO PARK Sports reserves the right to refuse participation if deemed unsuitable.

    課程內容 Course Content
    課程會視乎情況而定，有機會調整上課內容或時間表。
    Please note there are opportunities for adjustments to be made to the class schedule depending on the circumstances.


    惡劣天氣安排 Arrangements for Inclement Weather
    若於日營當日早上8時正仍懸掛紅色暴雨警告或三號颱風警告或更高警告訊號，日營將會取消。
    Day camp will be canceled if the Red Rainstorm Warning or No. 3 Typhoon Warning or higher warning signal is in effect on the camp day 8am.


    雨天安排 Rain Plan
    如遇天雨問題，若教練評估後認為不適合在戶外運動場地繼續活動，我們將安排課堂移至室內，按照實際情況決定進行原本的課堂內容或理論課或手工藝工作坊。
    In the event of rain, if the coach assesses that it is not suitable to continue activities at the outdoor venue, we shall arrange for classes to be held indoors, at which time a decision will be made regarding whether to proceed with the original course content, theoretical instruction, or DIY art workshops.

    每位參加者只可於報名時使用一組優惠碼，所有優惠不可重複或同時使用。

    為了培養參加者運動的興趣，我們推出「多報多優惠」的回贈優惠，參加者報名多於一天的日營將獲得不同價值的代金券。詳情如下:
    To cultivate participants' interest in sports, "Rebate Offer for Regular Class" is introduced. Participants who register for multple camp days will be entitled to vouchers of varying vaules. Details are as follows:

    • 報名2天的日營：$200常規課程代金券
      Participants who register for a 2-day camp will receive a $200 voucher for regular classes.
    • 報名3天的日營：$300常規課程代金券
      Participants who register for a 3-day camp will receive a $300 voucher for regular classes.
    • 報名5天或以上的日營：$600常規課程代金券
      Participants who register for a 5-day camp or more will receive a $600 voucher for regular classes.

    *曾報名參加GO!復活節運動探索營並參加本次日營即可獲得$100常規課程代金券回贈​
    *Participants who have joined GO! Happy Easter Sports Adventure Camp and enrol in this day camp will receive a $100 voucher for regular classes.

    所有代金券將會在整個活動完結後30個工作天內於發送到您的GO PARK Sports應用程式帳戶內，有效期為90天。代金券只限於報名GO PARK Sports常規課程時使用。
    All vouchers will be credited to the GO PARK Sports APP account within 30 working days after the activity period and are valid for 90 days. They can only be used for registration of regular GO PARK sports programs.

    Confirmation and Payment of Registration

    填妥報名表格及付款後，我們會在活動日開始前最少5天向您發送報名日營確認電郵。請確保提供有效的電郵地址。
    Upon completion of the registration form and payment, a confirmation email will be sent at least 5 days prior to the start of the event. Please ensure that a valid email address is provided.


    先到先得，名額有限，額滿即止。
    Quotas are limited and will be allocated on a first-come, first-served basis.

      By registering for the day camp from 6 July to 28 August, you can exclusively enjoy a one-night stay at ALVA Hotel by Royal for $1,000 (inclusive of a 10% service charge and 3% government tax). The room accommodates up to 4 guests and includes two queen beds.

      Breakfast: HK$100 per person per day (inclusive of a 10% service charge)

      • 帝逸酒店住宿體驗必須有家長或監護人陪同。
        The ALVA Hotel by Royal Staycation Experience requires participants to be accompanied by a parent or guardian.
      • 請注意，必須參與當天日營，才可附加當晚的住宿體驗。
        Please note that participation in the day camp on the designated day is required to be eligible for the overnight stay experience.
      • 參加者在填寫並提交需要帝逸酒店住宿體驗的報名表後，帝逸酒店職員將於五個工作天內聯繫參加者，確認酒店房間預訂資料並告知付款方式。在參加者付款後，必須收到帝逸酒店的確認電郵才算預訂酒店房間成功。
        After completing and submitting the registration form for ALVA Hotel by Royal staycation experience, hotel staff will contact the participant within five working days to confirm the room reservation details and provide payment instructions. The booking will only be confirmed upon receipt of a confirmation email from ALVA Hotel by Royal after payment has been made.
      • 已預訂酒店客房的參與者必須在酒店工作人員與您確認預訂後的48小時內完成付款；否則，酒店預訂將被視為失效。
        Participants who have reserved hotel rooms must complete payment within 48 hours after confirmation by hotel staff; otherwise, the reservation will be deemed invalid.
      • 酒店房間有限，售完即止，先到先得。
        Hotel rooms are limited and subject to availability on a first-come, first-served basis.

        Participation in activities requires individuals to be in good health condition, possess a reasonable level of fitness, and demonstrate a willingness to participate
      • 參加者及其家長必須理解是次活動所涉及的風險及所需之體能負荷，同時有責任確保參加者身體狀況 在不需要醫療輔助或其他輔助下，適合參與並能完成活動。
        Participants and their parents or guardians must acknowledge and understand the risks associated with this activity, as well as the physical demands involved. Parents or guardians are also responsible for ensuring that the participant is in a suitable physical condition for participation and able to complete the activities without requiring medical or additional assistance.
    • 參加者是否有任何物質過敏情況（例如：食物過敏、藥物過敏等）？ Does the participant have any known allergies (e.g., food allergies, drug allergies, etc.)?*
