We're so excited to have you join us as we revive a legacy that began in 1992- bringing authentic Samoan, Tahitian, Tongan, Fijian, Māori, and Hawaiian dance, music, and storytelling back to Utah and beyond.

MEDICAL RESPONSIBILITY: I understand that Polynesian Arts does not provide medical or accident insurance. I agree to maintain personal health insurance for the participant and accept financial responsibility for any medical expenses incurred. In the event of an emergency, I authorize Polynesian Arts staff to seek medical treatment for the participant if I cannot be reached.
Section 5: Agreements & Waivers
POLYNESIAN ARTS — ENROLLMENT AGREEMENT

Please read the full agreement below. Your signature at the end confirm agreement to all sections.

1. Tuition & No-Refund Policy
Monthly tuition is determined by membership category (Standard or Sponsored) and will be confirmed at the time of payment. Tuition is due on the 1st of each month; a $5 late fee applies to payments made after the 5th. Tuition covers instruction, studio rental, and general operational costs, and does not include costumes, accessories, practice uniforms, recital venue fees, or special event costs. All tuition, costume, production, and fundraising payments are non-refundable, including in cases of withdrawal, dismissal, or inability to perform, as Polynesian Arts commits funds, venues, staffing, and production costs in advance for each enrolled dancer. Accounts not kept current may result in suspension or removal from class.

2. Liability Waiver
I understand that participation in classes, rehearsals, performances, and related activities involves inherent physical risks, including slips, trips, and falls; muscle strains or sprains; contact with other participants; costume, prop, or fire knife–related injuries; and travel-related incidents. I voluntarily assume all risks associated with participation. In consideration of being allowed to participate, I release, waive, and discharge Polynesian Arts, New Beginnings Academy, and their owners, directors, instructors, staff, volunteers, and contractors, as well as any partner studios or event venues, from any and all claims, damages, injuries, losses, or liabilities arising from participation in any Polynesian Arts activity.

3. Photo & Video Release
I grant Polynesian Arts permission to photograph and/or record the participant during classes, rehearsals, performances, and events, and to use these images, video, or audio in social media, website content, promotional materials, flyers, advertisements, and archival or marketing content. No compensation will be provided for use of these materials, and Polynesian Arts will not use any media in a way that is inappropriate or harmful to the participant.

4. Conduct Expectations
The participant agrees to follow instructor directions and studio policies, treat staff and fellow dancers with respect, attend classes regularly and arrive on time, and represent Polynesian Arts with good character at all times. Polynesian Arts reserves the right to remove any participant for disruptive behavior, repeated absences, or failure to follow policies. Removal does not result in a refund.
