Parent/Guardian Consent, Safety & Media Release
By signing below, I acknowledge and agree to the following:
Supervision & Safety
I understand that my child will participate in a supervised KAPD Cheer event under the direction of KAPD Cheer coaches, staff, and designated volunteers. While reasonable care and supervision will be provided, I acknowledge that participation in group activities carries inherent risks.
I release and hold harmless KAPD Cheer, PRO Found Inc., its directors, staff, volunteers, and affiliates from liability for minor injuries, accidents, or unforeseen incidents that may occur despite reasonable precautions.
Medical Authorization
In the event of illness or injury, I authorize KAPD Cheer staff to seek appropriate medical care for my child. I understand that I am financially responsible for any medical expenses incurred.
Personal Belongings
KAPD Cheer and PRO Found Inc. are not responsible for lost, stolen, or damaged personal property, including but not limited to cell phones, electronics, jewelry, clothing, or other valuables. Bringing such items is at the family’s own risk.
Behavior Expectations
I understand that respectful, safe behavior is required at all times. If my child engages in unsafe or disruptive behavior, I may be contacted and required to pick them up early.
Photo, Video & NIL (Name, Image & Likeness) Release
I grant KAPD Cheer and PRO Found Inc. permission to photograph, record, and/or film my child during this event. I understand that images, video, and audio recordings may be used for promotional, educational, fundraising, or social media purposes, including but not limited to websites, printed materials, digital communications, and public presentations.
I grant permission for the use of my child’s name, image, and likeness (NIL) in connection with KAPD Cheer and PRO Found programming. I understand that no financial compensation will be provided for such use.
If I do not wish for my child’s image to be used, I will notify the organization in writing prior to the event.
Pick-Up Policy
I understand that my child must be signed out by an authorized adult at the designated end time and that timely pick-up is required.
