This Workshop Registration Form is for GVQC Members.
TO REGISTER: Select the workshop or workshops you would like to attend.
COST: One day workshop - $60, two-day workshop - $120. Total will automatically be calculated.
After selecting the workshop(s), select method of payment to be redirected to a secure payment portal. Please print the confirmation email as your record of registration.
LOCATION AND TIME: Registered participants will receive location information prior to the workshop.
REFUND POLICY: Should you need to withdraw from a workshop, contact the Registrar, Jenna Darlak, by phone only at 585-586-8708. Full refund will be made if withdrawal is made at least 30 days prior to the workshop. If you withdraw less than 30 days prior to the workshop and there is someone on the waiting list, you will be given the name of the person. It is your responsibility to contact that person to arrange your reimbursement. It there is no one on the waiting list, it is your responsibility to find a replacement for the class and notify the registrar of the replacement. All financial arrangements are between the two parties. If you cannot find a replacement, (1) you must notify the registrar, and (2) you will receive a voucher that must be used for another class within six (6) months.
