In consideration of Gr8 Elite , myself or minor child/ward being allowed to
participate in any way in/with the organizations named above, related events and activities, I being legally authorized, do hereby my signature below agree that the organization(s) named above have the unrestricted and exclusive right and permission, free from approval or review, to copyright and use in all media now or hereafter known, including but not limited to, pictures and videos of myself ,or my child/ward which he/she may be included intact or in part for promotion or other commercial use.

• Registration Cost is $250
• Helmet Rental is $100 
• NO REFUNDS
• This is for both returning and new players
• Returning players will have current helmet switch out
• Registration Closes Aug 1st *no exceptions *
• If your child is not PAID IN FULL they will not be eligible to play events
• All players need to have state Ids this is mandatory when playing this season.
• The league will not be accepting Birth certificate cards
• Physical Form Must be submitted by Aug 1st
