All events/programs are taught/supervised by trained and qualified instructors. I am fully aware of and appreciate all the risks, including the rise of catastrophic injury, paralysis, and even death, as well as other damages and losses associated with the participation of activities and other programs. With full consciousness of the risk involved and intending to be legally bound, I herby release The National Athletic Sports Arena, its directors, officers, employees and agents of any and from liability for any and all damages, injuries suffered in connection with The National Athletic Sports Arena. Further, it is affirmed that sufficient insurance covering all such injuries and damage shall be in full force and effect throughout the event by my insurance company.
