By registering my child for the Christmas Tree Soap Making Workshop at New Moon Art Studio, I acknowledge and agree to the following:

Participation & Assumption of Risk

I understand that my child will participate in a hands-on, instructor-supervised soap-making workshop involving age-appropriate creative activities, including selecting colors, adding decorations, mixing soap ingredients, and creating Christmas tree-shaped soaps.

I understand that participation may involve ordinary risks associated with craft activities, including minor skin irritation, allergic reactions, spills, and contact with soap-making materials or equipment. I voluntarily authorize my child to participate.

Soap-Making Activities & Safety

I understand that the workshop may involve soap bases, colorants, fragrances, glitter, decorative materials, molds, and other soap-making supplies.

New Moon Art Studio instructors will provide appropriate supervision and safety instructions throughout the workshop. Any heating or handling of hot materials will be performed by instructors or under their direct supervision.

I agree that my child will follow all safety instructions provided by the instructors.

Allergies & Skin Sensitivities

I understand that certain soap-making ingredients, including fragrances, dyes, and decorative additives, may cause allergic reactions or skin irritation.

I agree to notify New Moon Art Studio before the workshop of any known allergies, skin sensitivities, or other relevant health concerns that may affect my child's safe participation.

I understand that New Moon Art Studio cannot guarantee that workshop materials are free from potential allergens.

Handmade Soap & Take-Home Products

I understand that my child will create handmade decorative soaps to take home after the workshop.

I acknowledge that the finished soaps are handmade craft products and may vary in appearance, texture, and quality. New Moon Art Studio does not guarantee any specific result.

I understand that the soaps are intended for external use only and must not be ingested. I accept responsibility for supervising my child's use of the finished products after the workshop.

Release of Liability

To the fullest extent permitted by applicable California law, I release and hold harmless New Moon Art Studio, its owners, instructors, employees, volunteers, and representatives from claims arising from ordinary participation in the workshop, except to the extent liability cannot legally be waived, including claims involving gross negligence, reckless misconduct, or intentional wrongdoing.

Emergency Medical Authorization

If an emergency occurs and I cannot be reached promptly, I authorize New Moon Art Studio staff to seek appropriate emergency medical assistance for my child.

I understand that I am responsible for any resulting medical expenses to the extent permitted by law.

Acknowledgment & Consent

By checking the box below and submitting this registration, I confirm that:

I am the child's parent or legal guardian.
I have read and understood this consent and waiver.
I have disclosed any known allergies or relevant sensitivities that may affect my child's participation.
I understand the nature of the soap-making activities and their associated risks.
I voluntarily authorize my child to participate in the Christmas Tree Soap Making Workshop.
