Assumption of risk and release
Scope. Kuntanawa Nation events are religious and ceremonial in nature, offered as spiritual guidance within the Kuntanawa tradition. They are not intended to diagnose, treat, cure or prevent any disease, illness, or medical or psychological condition, and are not a substitute for licensed medical, psychological, psychiatric or emergency care. Your responsibility. You remain responsible for your own healthcare decisions and should continue consulting appropriately licensed providers regarding medical concerns, diagnoses, medications or treatment. Emergency services. Kuntanawa Nation does not provide emergency services or crisis intervention. If you are experiencing a medical emergency or are at immediate risk of harm, contact your local emergency services. Discretion. Facilitators may decline, pause, postpone or end your participation at their sole discretion. Assumption of risk. You participate voluntarily and at your own risk. In consideration of being permitted to participate, you act for yourself, your executors, administrators, heirs, next of kin, successors and assigns.

Release

I waive, release and discharge Kuntanawa Nation, its facilitators, staff and hosts from any and all liability, claims and demands arising from my participation, on behalf of myself and my heirs, executors, administrators, next of kin, successors and assigns.
Indemnity

I indemnify and hold harmless Kuntanawa Nation, its facilitators, staff and hosts from any claims made by a third party arising from my participation, and I covenant not to sue in connection with my participation.
Membership
By completing this form you become a short-term member of Kuntanawa Nation's Circle of the Stars for the duration of this event, and are held within the association while working with Kuntanawa Nation sacred medicines. You are joining as a private individual, for your own reasons. You are not joining on behalf of, or at the direction of, any organization seeking to investigate, record or gather information about the association, its members or its practices. Membership may be revoked at any time by the Trustees or their designee. If you would like to support Kuntanawa Nation as a longer-term member, we'll send information after this event.

We photograph and film at our events. These images document the work of the Kuntanawa people, support the mission, and are used to share what we do — including for the demarcation of Kuntanawa lands. You grant Kuntanawa Nation permission to use your image, likeness and voice without payment or other compensation. You understand these may be edited, copied, exhibited, published or distributed, including online, and you waive any right to royalties or other compensation. This release applies to recordings made at this event.
