  • Basketball Open Gym Liability Waiver, Informed Consent and Acknowledgement


    I understand that participation in basketball open gym sessions and pickup scrimmage games involves physical activity and inherent risks, including but not limited to collisions with other players, falls, sprains, strains, fractures, serious injury, paralysis, or death.

    By signing this waiver, I voluntarily choose to participate and assume the risks associated with these activities. To the extent permitted by law, I release and hold harmless the organization hosting the open gym, its owners, employees, coaches, representatives, and affiliates from claims or liability for injuries or damages arising from my participation, except where liability cannot legally be waived.

    I understand that I am responsible for determining whether I am physically able to participate safely. I agree to follow all facility rules, respect other participants, and conduct myself in a safe and sportsmanlike manner.

    If the participant is under 18 years of age, I confirm that I am the participant’s parent or legal guardian and give permission for the minor to participate in basketball open gym sessions and pickup scrimmage games. I acknowledge the risks involved and agree to the terms of this waiver on behalf of the minor, to the extent permitted by law.
