WARNING OF RISK AND ASSUMPTION OF RISK
I voluntarily choose to participate as a vendor in the craft vendor fair identified above, including loading, unloading, setup, displaying and selling merchandise, demonstrations approved by the District, and cleanup. I understand that these activities involve risks, including slips, trips and falls; lifting injuries; vehicle traffic during loading and unloading; falling or unstable displays; electrical cords and equipment; contact with other participants; and loss, theft or damage to merchandise, money and personal property. Outdoor activities may also involve wind, rain, extreme temperatures and other weather conditions. These risks may result in property damage, serious injury or death.
I acknowledge that all risks cannot be eliminated. To the fullest extent permitted by Illinois law, I voluntarily assume the risks of injury, damage or loss arising from my participation in these activities, including risks arising from ordinary negligence of the parties released below.
WAIVER AND RELEASE OF LIABILITY
To the fullest extent permitted by Illinois law, I waive my claims and release Summit Park District, its commissioners, officers, employees, agents and volunteers from liability for injuries, death, damages or losses that I sustain arising out of my participation in the activities described above, including claims based on their ordinary negligence. This release does not apply to willful and wanton misconduct or other liability that cannot lawfully be waived.
VENDOR RESPONSIBILITIES AND INDEMNIFICATION
I am responsible for maintaining a safe vendor area, securing displays and merchandise, keeping aisles and exits clear, and following District rules and staff instructions. I will obtain applicable permits and licenses and comply with applicable requirements for the products I sell. I will provide proof of insurance and additional insured documentation when required by the District; insurance types and limits are subject to District approval.
To the fullest extent permitted by Illinois law, I agree, individually or on behalf of the business identified above if authorized, to defend, indemnify and hold harmless Summit Park District, its commissioners, officers, employees, agents and volunteers against third-party claims, damages, liabilities and reasonable legal expenses to the extent caused by my or the business’s negligent acts or omissions in vendor operations, products, displays or equipment, or those of persons working for the business. This obligation does not extend to claims caused by the District’s own negligence or willful and wanton misconduct.

Requests received fewer than five calendar days before the event are not eligible for a refund, and the registration fee will be forfeited. No refunds will be issued for failure to attend.
Vendor Space Transfers: Vendor spaces may not be transferred to another vendor without prior District approval.
Event Cancellation: If Summit Park District cancels the event, registered vendors will receive a full refund of their registration fee. The District will not reimburse other vendor expenses.
