- 組別：
  成人英文組、成人中文組、青少年組及兒童組。
  
  住宿與餐飲：
  可選擇住营内 2 晚（6餐）, 3 晚（9餐）或住营外周五( 1餐 ),周六( 3餐 ),主日( 2餐 )
  
  青少年報名：
  所有青少年組參加者均須繳交完整的 3 晚費用，即使提前離開亦不例外。青少年組需使用另外的報名表報名。
  
  責任豁免書：
  所有參加者在抵達前都必須完成責任豁免書。報名表中將提供相關連結。
  
  支付方式：
  在此注册表格底部，选择借记卡/信用卡或其中一种数字支付方式。
  
  報名截止日期：

  Important Details:
  
  Tracks Available: Adult English, Adult Mandarin, Youth, and Children.
  
  Lodging & Meals: Options are available staying at camp for 2-night (6 meals) or 3-night (9 meals) or commuting on Fri (1 meal), Sat (3 meals) and/or Sun (2 meals).
  
  Note: All Youth track registrations require the full 3-night registration fee, even if departing early. Youth registration is on a seperate form.
  
  Waivers: All attendees must complete the liability waiver before arriving. A link will be provided on this form.
  
  Payment Methods: At the bottom of this registration form, select debut/credit card or one of the digital payment methods.

  美东南华人基督徒冬令会儿童营活动及医务免责书
  
  Activity Waiver: In consideration of my child being allowed to participate in activities at the Shocco Springs Children’s Camp from December 31, 2026 to January 3, 2027, sponsored by Huntsville Chinese Christian Church and Nashville Chinese Baptist Church, I hereby release, discharge, and agree to hold harmless both churches, including their ministers, officers, employees, agents, and all volunteer personnel from any liability, claims, demands, and causes of action arising from any loss, damage, injury, or illness sustained by my child during the camp. I also agree to indemnify both churches and their representatives from any claims, damages, and liabilities (including reasonable attorney fees, litigation expenses, and court costs) that are required to pay arising from any injury or damage sustained during the camp activities.
  
  Medical Waiver: In the event of a medical emergency, I authorize the staff and volunteers of Huntsville Chinese Christian Church and the Nashville Chinese Baptist Church to arrange for medical treatment deemed necessary for my child's well-being. I release both churches and their representatives from any liability related to medical care provided. I accept all financial responsibilities for medical expenses incurred during emergencies and understand that efforts will be made to contact me in the event of a serious illness or injury.

- [Please Note] Youth Retreat starts on 12/31/2026 and the Adult Retreat starts on 01/01/2027. The adult retreat campers may opt for a 3-night package if they wish to start camp on the same day as the Youth Retreat.
  
  For campers staying for 3 nights: There will be no childcare/children camp service before the adult camp starts after dinner on 01/01/2027. Please take care of your own minor child/children before then.
