Indemnity:
I, the parent/ guardian of the registrant, a minor, acknowledge that participating in sport activities carries certain inherent risks, including but not limited to injury, illness, and death. By submitting this form, I hereby release, discharge and/or otherwise indemnity Summit Soccer Schools, its volunteers, staff, organization, sponsors and any affiliated person to Summit Soccer Schools from any and all claims, demands, damages, liabilities and expenses on behalf of the registrant as a result of the registrants participation in our programs.
Consent for Pictures and Videos:
I, the parent/guardian of the registrant, a minor, provide consent to Summit Soccer Schools to use pictures and videos of registrant for advertising purposes o social media only. If a parent/ guardian or registrant does not wish to have their image to be used in this manner, they may request to have their image be excluded.
By submitting this form, I, as the parent/ guardian of the registrant, a minor, agree that I and the registrant agree with the terms and conditions of Summit Soccer Schools.
