Fall Festival Vendor Registration Form
Justin Elementary School PTA is hosting the 4th Annual Fall Festival on Thursday, October 29th 4:30-6:30pm. We invite local businesses and community members to participate in this free event. There is NO COST to participate! We do ask that you please donate an item/service to our Fall Fest raffle and consider donating 10% of your proceeds should you be selling anything at the event. Thank you for being an amazing part of our community!
Donations
The Justin Elementary PTA is a Registered 501(c)(3) Tax Exempt Organization. Upon receipt of your donation, you will be provided a form with the tax identification number for your records. All donations will be used solely for the Fall Festival event unless stated otherwise. The PTA does not retain items or monies for individual use. Should a monetary donation not be used for a specific purpose, it will be added to the event budget.

In lieu of traditional booth fee, participating vendors are asked to donate 10% of their total sales made during the fall festival to Justin Elementary PTA. *Auction donations are optional and separate from the 10% sales contribution.
