By registering my child for Dream Big Toddler Ball, I understand that participation in basketball and related activities involves a risk of injury. I voluntarily allow my child to participate and release Dream Big Basketball, its coaches, volunteers, staff, and facilities from liability for injuries or accidents that may occur during the program, except where prohibited by law. I authorize emergency medical care for my child if I cannot be reached.

I also give Dream Big Basketball permission to photograph and/or record my child during practices, games, and events and to use those images or videos for program-related promotional purposes, including social media, flyers, and the Dream Big Basketball website.
