Cave City Youth Basketball Registration Form (2026)

Thank you for your interest in the Cave City Booster Club youth basketball program. Registration forms and payment are due by October 10th at noon (12:00pm). Each athlete will be provided a shirt included in the registration cost. Registration fees are NON-REFUNDABLE.

RELEASE STATEMENT: I, the parent/guardian of this student, a minor, agree that the student and I will abide by the rules of the Cave City Booster Club, its affiliated organizations, and sponsors. We recognize the possibilities of physical injury in this activity and hereby release, discharge, and/or indemnify all involved against any claims by or on behalf of the student's participation in this program.
CONDUCT STATEMENT: I understand that if I do not conduct myself in a way that represents the Cave City community and the Cave City Booster Club well, I will be asked to refrain from attending practices and/or games for the remainder of the season or the student may be removed from the team if the Cave City Booster Club feels your conduct/behavior may be harmful to others. I am aware that violations of the code of conduct include the following: 1. Yelling at or ridiculing players for mistakes. 2. inciting or participating in unsportsmanlike behavior(s). 3. Attending events under the influence of substances. 4. Failure to ensure an environment free of profanity, drugs, and alcohol while attending practices/games.
CONSENT FOR MEDICAL TREATMENT: I hereby give consent for emergencies prescribed by a duly licensed Doctor of Medicine or Dentistry. This care may be given via whatever means are necessary to preserve life, limb, or well-being.
