YUVA Garba Workshop Registration & Payment

 By registering my child for the Garba Workshop conducted by YUVA Dance Academy , I acknowledge and agree to the following:

1. Participation and Acknowledgment of Risk

I understand that dance and movement activities involve physical activity and carry inherent risks, including slips, falls, collisions, muscle strains, or other injuries.

I confirm that I/my child may participate in age-appropriate physical activities. I agree to disclose information reasonably necessary for the safe participation of my child, including relevant allergies, medical considerations, or physical limitations.

2. Assumption of Risk & Release of Liability

To the extent permitted by applicable law, I release and hold harmless YUVA Dance Academy, its instructors, staff, representatives, volunteers, and venue partners from claims arising from the ordinary and inherent risks associated with participation in the program.

Nothing in this agreement is intended to waive rights or liability that cannot legally be waived.

3. Emergency Medical Care

In the event of an illness, injury, or medical emergency, I authorize YUVA Dance Academy, its representatives, to contact emergency services and/or the emergency contact provided if I cannot be reached. I understand that I am responsible for any resulting medical expenses.

For participants under 18: I confirm that I am the participant’s parent or legal guardian, or am otherwise legally authorized to provide this consent.

4. Student Safety & Conduct

To maintain a safe, respectful, and enjoyable workshop, participants agree to:

• Follow instructions provided by YUVA Dance Academy staff.
  Treat instructors, staff, volunteers, venue personnel, and other participants respectfully.
• Wear comfortable, appropriate clothing and footwear suitable for Garba/dance.
• Use dandiyas/props responsibly and only as instructed.
• Avoid unsafe, disruptive, threatening, or inappropriate behavior.
• Respect the venue, its equipment, and other participants’ belongings.
• For participants under 18, follow the workshop’s check-in, dismissal, and pickup procedures.
  
  YUVA Dance Academy reserves the right to ask a participant to leave if their behavior creates a safety concern or significantly disrupts the workshop. Removal for conduct violations may be without refund.

 

5. Registration, Cancellation & Refund Policy

Registration is valid only after payment is successfully completed.

Workshop registrations are non-refundable, except if YUVA Dance Academy cancels the workshop. Registrations may be transferred to another eligible participant with prior approval from YUVA Dance Academy.

If the workshop must be rescheduled due to circumstances beyond our reasonable control, registration will be transferred to the rescheduled date. YUVA will communicate any available alternatives if a participant cannot attend the new date.

 6. Drop-Off, Pickup procedures

Parents/guardians are responsible for ensuring that the authorized persons pickup their child if the participant is a child.

7. Personal Belongings

Students should avoid bringing unnecessary valuables or personal belongings to class.

YUVA Dance Academy is not responsible for lost, misplaced, or damaged personal items except to the extent required by applicable law.

 

By signing below, I acknowledge that my electronic signature represents my acceptance of the acknowledgments, waiver, rules, and policies stated above.
