Participation and Liability Acknowledgment

I am the parent or legal guardian of the athlete named on this form, and I give permission for my child to participate in the selected Five Star Athletics fall program.

I understand that cheer, tumbling, and dance involve physical activity and may include falls, collisions, and other risks of injury. I will share any medical condition, allergy, injury, or accommodation that Five Star Athletics should know about and will update the gym if that information changes.

If I cannot be reached in an emergency, I authorize Five Star Athletics staff to seek appropriate emergency medical care for my child. I understand that I am responsible for any resulting medical costs.

To the fullest extent permitted by Massachusetts law, I release Five Star Athletics and its owners, coaches, and staff from claims arising from ordinary negligence connected with my child’s participation. This release does not apply to conduct that cannot legally be released.
