WEDNESDAY: Spike Junior Coaching

Please use this form to sign your child up for our weekly junior coaching sessions, Wednesday's 5:15-6:40pm for $16 per session. As spots in this coaching group are limited we will only be offering the full sign up for term 4, which will be the remaining 7 sessions this term due to the shcool using the hall for end of year events.

Players will master the basic skills and develop their game sense. The age range of this group is age 11-13 however this is a guidleline for us to determine skill & player experience. If your child is outside of this age range please make sure you have reached out to Spike to discuss whether this program is right for your child. 

 *Please note any missed sessions will not be reimbursed due to the high demand of the program*

*Sesssions closures are October 14th. November 18th*

Please note that Spike does not have personal accident insurance cover for players. If your child is injured as a result of an accident or incident while participating in volleyball activities organised by Spike, all costs associated with the injury, including medical costs are the responsibility of the parent/carer. It is up to all parents/carers to decide what types and what level of private insurance they wish to arrange to cover their child. Please take this into consideration in deciding whether or not to allow your child to participate in Spike Volleyball.

By signing this form / agree that: - Spike does not have personal accident insurance cover for players. - In the event of an accident or illness, Spike staff may obtain or administer any medical assistance or treatment my child may reasonably require. - give consent for my child to participate in Spike Volleyball.

I grant permission to Spike Volleyball Association for the use of photograph(s) or electronic media images taken of my child listed above for any advertising purposes. I understand that I may revoke this authorization at any time by notifying Spike Volleyball Association in writing. The revocation will not affect any actions taken before the receipt of this written notification. Images will be stored in a secure location and only authorized staff will have access to them. They will be kept as long as they are relevant and after that time destroyed or archived.
