2027 GBCYFL SPRING FLAG Football Registration Form

IMPORTANT REGISTRATION INFORMATION – PLEASE READ CAREFULLY. ** A separate registration form must be completed for EACH child. ** Once an age group reaches capacity, registration for that division will CLOSE. No waitlists or exceptions unless announced by the league. ** Team requests, coach requests, or practice day requests will NOT be accepted or honored. ** All teams will be formed through a league draft process in February 2027 to ensure competitive balance. ** Practice schedules will be finalized and communicated to families no later than 3/1/2027. ** Game schedules will be released around the same time. ** GBCYFL reserves the right to combine grade divisions if registration numbers are insufficient to support a standalone division. If divisions must be combined, all teams within the affected divisions will be formed through a fair and balanced draft process. ** By submitting this registration, you acknowledge and agree to the above policies.

SPRING FLAG FOOTBALL PARTICIPATION WAIVER & ACKNOWLEDGEMENTS

**Participation, Risk & Liability**

I understand that participation in flag football involves inherent risks, including but not limited to physical injury, falls, collisions, or other unforeseen incidents.

I voluntarily assume all risks associated with my child’s participation and release and hold harmless the league (GBCYFL), its officers, directors, coaches, volunteers, sponsors, and facilities from any liability, except in cases of gross negligence.

**Basic First Aid Authorization**

I authorize league officials, coaches, or designated volunteers to provide basic first aid to my child in the event of a minor injury or illness during practices, games, or league activities.

Basic first aid may include cleaning and bandaging minor wounds, applying ice, and providing initial care until a parent/guardian or emergency medical personnel arrive.

** Photo & Video Release**

I grant permission for the league (GBCYFL) to photograph or record my child during league activities and to use such images or videos for league-related purposes, including social media, promotional materials, and the league website, without compensation.

** Refund Policy**

I understand that all registration fees are non-refundable once registration is completed.

No refunds will be issued for any reason, including but not limited to scheduling conflicts, team placement, practice times, weather-related changes, injury, or voluntary withdrawal.

** Weather & Schedule Changes**

I understand that practices and games may be delayed, rescheduled, relocated, or canceled due to weather, field conditions, or other circumstances. Schedule changes do not qualify for refunds.

** Communication Policy**

I understand that all official league communication will be conducted through the Band app and/or email, and it is my responsibility to ensure my contact information is accurate and to regularly monitor league communications.

** Code of Conduct**

I agree that my child, family members, and spectators will conduct themselves in a respectful and sportsmanlike manner at all league (GBCYFL) activities. I understand that unsportsmanlike or disruptive behavior may result in removal from league activities and, if necessary, dismissal from the league without refund.
