🎃 theProject. Halloween Bash — Participant Waiver & Release
Acknowledgment of Risk
I understand that participation in the theProject. Halloween Bash involves physical activity and other activities that may carry inherent risks. Activities may include, but are not limited to, dancing, movement activities, games, crafts, costume-related activities, and other party activities.
I understand that potential risks may include falls, collisions, muscle strains, sprains, bruises, cuts, or other injuries. I understand that physical contact may occur during dancing, games, or other activities.
I certify that the participant is physically able to safely participate in the activities associated with this event and that I have disclosed any known physical limitations, allergies, or other conditions that may affect their participation.
🍭 Food, Treats & Allergies
I understand that snacks, treats, and/or beverages may be provided during the event. I understand that theProject. cannot guarantee an allergen-free environment.
I have provided accurate information regarding the participant's known food allergies or dietary restrictions and understand that I am responsible for determining whether food or beverages are appropriate for the participant.
🚑 Medical Authorization
In the event of an emergency, I authorize theProject., its owners, instructors, employees, volunteers, and designated representatives to seek appropriate emergency medical care for the participant if I cannot be reached or if circumstances require immediate action.
I understand that I am responsible for any medical expenses, treatment costs, or transportation costs incurred on behalf of the participant.
🕷️ Release of Liability
To the extent permitted by law, I voluntarily assume the risks associated with participation in the theProject. Halloween Bash and release theProject., its owners, instructors, employees, volunteers, and agents from claims arising from ordinary risks associated with participation in the event.
This waiver applies to activities occurring during the theProject. Halloween Bash on October 24, 2026, including dancing, games, crafts, snacks/treats, costume activities, and other event activities.
I understand that this agreement does not release or waive claims that cannot legally be waived.
