LIABILITY WAIVER, RELEASE, AND CONSENT TO MEDICAL TREATMENT
In relation to and in consideration of the acceptance of my entry or that of minor child(ren) provided to me or that of the minor child(ren) by Oahu Tae Kwon Do Center, INC., Oahu Taekwondo Center, OTC, its agents, owners, officers, volunteers, participants, employees, affiliate and subsidiary entities, and all other persons or entities acting in any capacity on its behalf, I hereby enter into this Participant Liability Release and Assumption of Risk Agreement and agree to waive, release, discharge, covenant not to sue, and agree to indemnify and save and hold harmless any and all rights and claims for damages which I may have or may accrue to me or my minor child(ren). I understand that all entry fees are nonrefundable. I believe that my experience and capabilities, or those of the minor child, qualify them to participate in the Oahu Taekwondo Tournament. I understand that participation in the Oahu Taekwondo Tournament activities involves risks and dangers of serious bodily injury, including permanent disability, paralysis, and death. These risks and dangers may be caused by myself or the minor child's own actions, or inactions, and/or the actions or inactions of others participating in the Oahu Taekwondo Tournament.
AS A COMPETITOR OR PARENT/LEGAL GUARDIAN OF THE COMPETITOR, I UNDERSTAND IN CASE OF INJURY, ONLY BASIC FIRST AID WILL BE AVAILABLE ON SITE, AND THAT I AM FULLY RESPONSIBLE FOR ANY OR ALL RESULTING MEDICAL OR OTHER EXPENSES, INCLUDING AMBULANCE SERVICES. I ALSO UNDERSTAND THE NEAREST EMERGENCY ROOM IS: THE QUEENS MEDICAL CENTER - WAHIAWA, LOCATED AT 128 LEHUA STREET, or PALI MOMI MEDICAL CENTER, LOCATED AT 98-1079 MOANALUA ROAD.
I understand that videos and or pictures provided by me, my child(ren), or any other identities not listed may be used on social media (Facebook/Instagram/Yelp), and or Webpage for publicity and or promotions. I waive all rights to any compensation.
I have read this release of liability and assumption of risk agreement, fully understand its terms, acknowledge that I am giving up substantial rights by signing it, and sign it freely and voluntarily without any inducement. I have read this release of liability and assumption of risk agreement.
