I understand that my child will participate in a children’s craft workshop involving activities and materials such as scissors, glue, paint, and other craft supplies. I understand that participation involves some inherent risks, including minor cuts, scrapes, allergic reactions, or other accidental injuries. I confirm that I have provided accurate information about my child’s allergies and relevant medical or safety needs.

I understand that reasonable precautions and supervision will be provided during the workshop. I voluntarily give permission for my child to participate and agree to release the workshop organizer from liability to the extent permitted by law for injuries or losses arising from the inherent risks of the activities, except where caused by the organizer’s negligence or other liability that cannot legally be excluded.
