CLE 2:16 is a day-long event for high school teens to gather in fellowship as the young Church, sponsored by the Office of Youth Ministry. Exploring Luke 2:16, CLE 2:16 will celebrate the parish year of Eucharistic Revival, with a focus on the connection between the incarnation and the Eucharist – how from the beginning the Lord came to feed us.

We will carpool from the St Helen parking lot meeting at 9am and return to St Helen around 10pm.

Consent Form and Liability Waiver

I, as the parent or legal guardian of the above child(ren) do hereby grant permission for my child to participate in the CLE 2:16 - Diocesan Youth Conference. I understand that my child(ren) will be transported to/from St. Helen Church to John Carroll University by Parents or Life Teen Core Members over the age of 21 years. Participants 18 years of age, may be transported by young adult volunteers who have been out of high school for at least one year, but who are not yet 21 years old.

I acknowledge that the aforesaid participant(s) will be involved in physical activities that may cause injury and that there may be risks foreseen and unforeseen. I agree by my signature to release, absolve, indemnify, and hold harmless St. Helen Parish (Newbury, Ohio), their employees and agents; The Roman Catholic Diocese of Cleveland; The Bishop of the Roman Catholic Diocese of Cleveland their successors and assigns from any and all liability for injury, medical fees, hospital bills, or doctor bills of aforesaid participant as well as all debts, claims, demands, costs, expenses, damages, actions, and causes of actions -- I waive all claims of any kind against those mentioned.

Authorization for Medical Treatment

I, as parent or legal guardian of the above child(ren) do hereby give my consent for St. Helen Parish (Newbury, Ohio) staff and the chaperones, or other adult representative, in the event that all reasonable attempts to contact me have been unsuccessful, to seek medical attention and treatment deemed necessary by medical personnel. I give my permission to transfer my child to the nearest hospital.
