Required Participant Consent
Medical Responsibility: I understand that participation in the Fayetteville Liberty team tryout involves physical activity and may involve risk of injury. I acknowledge that I am responsible for determining whether I am medically and physically able to participate and accept responsibility for my own medical needs and expenses associated with my participation.
Travel & Transportation Responsibility: I understand that I am solely responsible for arranging and paying for my transportation and travel to and from the Fayetteville Liberty team tryout.
No-Refund Policy: I understand that the $75 tryout registration fee is non-refundable, including if I am unable to attend, withdraw, or am not selected for the team.
Participation & Injury Acknowledgment: I understand that basketball tryouts involve strenuous physical activity and voluntarily accept the ordinary risks associated with participating.
Photo & Video Consent: I give the Fayetteville Liberty permission to photograph and/or record me during the tryout and to use that content for team marketing, social media, promotional, and documentary purposes.
Tryout/Selection Acknowledgment: I understand that registering for or participating in the tryout does not guarantee a roster spot, contract, compensation, or future employment with the Fayetteville Liberty.
