Victory United Fellowship of Churches 2026 Youth Registration Form
Proverbs admonish us to train up a child in the way he should go: and when he is old, he will not depart from it. At Holy Convocation 2026, dedicated, fun and interactive class sessions are available for our children ages 5-18 years old. Holy Convocation Youth offers three entertaining days of bible-based lessons and activities especially created for our young leaders. The registration fee is $25.00 per child.

Parental Consent:
I, the undersigned parent/guardian, give permission for my child, named above, to participate in Youth Convocation 2026 and all scheduled activities associated with this event.

Medical Emergency Authorization:
In the event of an accident, illness, or emergency where I cannot be reached, I authorize a representative of the Victory United Fellowship of Churches to consent to any necessary medical treatment for my child. I accept responsibility for any medical expenses incurred.

Liability Waiver:
I acknowledge that participation in youth activities involves certain risks. By signing below, I release and hold harmless Victory United Fellowship of Churches, Victory City Church, its leaders, staff, volunteers, and affiliates from any liability, claims, or demands for personal injury, illness, or property damage that may occur during this event, except in cases of gross negligence or willful misconduct.
