Soccer is a physical sport that involves the risk of injury. I assume all risks and hazards associated with my participation in College Soccer ID’s activities. I certify that I am in proper physical condition to participate in soccer practices and games and have no illness, disease or existing injury or physical defect that would be aggravated by my participation in these activities. I will inform College Soccer ID in writing if this status changes.
I have a current, active, personal injury insurance policy in force, which covers my participation in College Soccer ID activities. Under any condition, I am wholly responsible for any and all medical expenses arising from my participation in College Soccer ID’s activities, both in practices and games and while travelling to and from these events. I have the right and responsibility to inspect the equipment and facilities prior to all events and, if I believe that anything may be unsafe, I will advise the coach or supervisor of the condition and may refuse to participate. Participation is at player’s own risk.
I authorize my photograph, picture or likeness, and voice to appear in any documentary, promotion (including advertising), television, video, or radio coverage of College Soccer ID.
I hereby RELEASE, WAIVE LIABILITY, AGREE TO HOLD HARMLESS AND INDEMNIFY and COVENANT NOT TO SUE, the College Soccer ID, Athlete Training and Health facility, British International School facility, Shadow Creek Sports Complex, City of Pearland, City of Houston Parks, Crossbar Academy, Sporti USA, Lehi Sports Park and any other facilities we utilize, their associated directors, administrators, officers, managers, employees, coaches, trainers, volunteers, sponsors and advertisers, and other agents, estates or executors (“Indemnified Parties”), from any and all liability incurred and my participation in College Soccer ID's programs. This RELEASE AND INDEMNITY AGREEEMENT includes owners, lessors, and lessees of premises where practices and/or matches occur. This RELEASE and INDEMNITY AGREEMENT set forth above is intended to and does hereby RELEASE and INDEMNIFY the Indemnified Parties against the consequences of their own real or alleged joint or concurrent negligence in connection with College Soccer ID's programs.
I have read this document and fully understand its contents. I acknowledge that I have given up substantial rights by accepting this document and that I do so voluntarily. My signature attests to this on behalf of myself and my executors, personal representatives, administrators, heirs, next-of-kin, successors for the benefit of College Soccer ID and Indemnified Parties.
