SNIPTEMBER VOUCHERS ARE LIMITED TO ONLY ONE PET PER HOUSEHOLD. AT THIS TIME WE ARE UNABLE TO ISSUE VOUCHERS TO PET OWNERS WHO HAVE ALREADY USED THIS SERVICE.

Vouchers are for owned pets only and are only good for the owner and pet listed and may not be transferred to another person or pet.

All dogs/cats in the home must be spayed or neutered or be scheduled within one month of requesting assistance.

Once approved, you will be mailed a voucher with directions on how to schedule your appointment. ONLY SCHEDULE YOUR APPOINTMENT ONCE YOU RECEIVE THE VOUCHER IN THE MAIL. The application is not the voucher and we are not responsible for payment on a surgery that is performed without a voucher.

If the animal is not current on a rabies vaccine given by a licensed veterinarian applicants are responsible for that cost at the time of surgery. Rabies are included at Eastridge Animal Hospital and El Dorado Animal Hospital. A discounted cost of $10 for rabies will be applied at Camden Animal Hospital.

Vouchers are good for two months from the date listed on the voucher.

Applicants can pay through this form or can mail a check in with SNIP in the memo line. Our mailing address is PO Box 1526, Magnolia, AR 71754. We will not review applications unless payment has been received. If your application is denied, your money will be returned along with an explanation of why SNIP will be unable to assist you.

Please allow up to two to three weeks for applications to be reviewed once they are received. We are all volunteers, so your patience is appreciated.

By signing this application you agree to pay $40 for spay or neuter services. Other options, including bloodwork or additional vaccines, are the responsibility of the pet owners and not SNIP. Additional charges for animals that are pregnant or in heat are the responsibilty of the pet owner and not SNIP.

If you have questions about how the surgery will affect your pet contact Eastridge Animal Hospital, Camden Animal Hospital, or El Dorado Animal Hospital, all SNIP partners. DO NOT CONTACT the vets offices for questions related to your application.

SNIP is funded by donations and grants. This program is for LOW INCOME PET OWNERS only.

At SNIP we believe that through education, action, and volunteering we can make a difference in the lives of dogs and cats. If you are benefitting from the SNIP program we ask that you help educate others on the importance of spaying and neutering. Volunteering at local animal shelters is free and another way to show your support. Volunteers are always needed to help clean, walk, play and socialize homeless animals.
