Acknowledgment of Waiver and Consent
TCMI Church Youth Group Outing

I, the parent/legal guardian of the participant listed above, give permission for my child to attend and participate in the TCMI Church Youth Night outing to Adventure Dome. I understand that participation may include amusement park rides, games, attractions, walking, and other recreational activities that involve inherent risks of injury. I voluntarily authorize my child to participate and agree to release and hold harmless TCMI Church, its pastors, staff, volunteers, leaders, and chaperones from claims arising from the ordinary risks associated with participation in this outing, to the extent permitted by law.

Emergency Medical Authorization

In the event of an emergency and I cannot be reached, I authorize TCMI Church staff or designated chaperones to obtain reasonable emergency medical care for my child. I understand that I am responsible for any medical expenses incurred.
