At the end of the form, you will pay the $69 CNA Renewal Assistance fee and submit your renewal packet to Faith Forward Healthcare Training Academy. Your renewal request is not considered submitted to Faith Forward until the form has been completed, the required Form 5506-NAR has been uploaded, the $69 fee has been successfully paid, and you click the final submit button.

WHAT HAPPENS NEXT?
Faith Forward will review your submitted CNA renewal packet and payment to confirm all required documents and information have been received. If anything is missing or incomplete, we will contact you and the renewal cannot move forward until the missing items are received. Once everything is received and verified, Faith Forward will prepare the renewal documents and assist with completing and submitting the CNA renewal application in TULIP for processing by the Texas state agency. After the TULIP renewal application has been successfully submitted, state processing generally takes about two weeks for approval and for the renewed CNA status to be updated on the Texas Nurse Aide Registry. This is only an estimated timeframe, and Faith Forward cannot guarantee the exact approval date. Please monitor your email and TULIP account for any state notices or requests for additional information. Once the renewal appears on the Texas Nurse Aide Registry, please verify that your renewed status is correct. Faith Forward will assist throughout the process, but final renewal approval and registry posting are controlled by the applicable Texas state agency.
