  • LIABILITY WAIVER & ASSUMPTION OF RISK
    By participating in classes, programs, rehearsals, performances, events, or other activities offered by Rhythm Enterprises LV LLC, DBA Rhythm Nation Dance & Fitness Studio (“the Studio”), I understand that dance, fitness, and movement activities involve inherent risks, including but not limited to falls, collisions, muscle strains, sprains, and other injuries.

    I voluntarily assume these risks for myself and/or my child(ren) participating in Studio activities. To the fullest extent permitted by law, I release and hold harmless Rhythm Enterprises LV LLC, DBA Rhythm Nation Dance & Fitness Studio, its owners, instructors, employees, independent contractors, and representatives from claims arising from ordinary risks associated with participation, except where liability cannot legally be waived.

    I understand that I am responsible for informing the Studio of any relevant medical conditions, allergies, injuries, or physical limitations that may affect safe participation. I understand that the Studio does not provide medical insurance for participants.

    In the event of an emergency involving my minor child and I cannot be reached, I authorize the Studio to contact emergency medical services and obtain reasonably necessary emergency care. I understand that I am responsible for any resulting medical expenses.

  • Photo & Video Release

    Rhythm Nation Dance & Fitness Studio may photograph or record classes, rehearsals, performances, events, and other studio activities. Please select whether you give permission for images or recordings of yourself and/or your participating child(ren) to be used for studio promotional purposes, including the studio website, social media, advertising, flyers, and other marketing materials.
