Create a personalized book plate to honor your child's special day for a $25 tax deductible donation.

LS Media Specialist, Mrs. Wigdor, will help your child select a favorite book from our library’s collection. Then, on or near the child’s special day, the birthday boy or girl will be presented with a festively decorated package that contains the library book with the personalized book plate inside, plus a birthday memento commemorating your contribution that can be taken home as a keepsake.

By completing this form and submitting a $25 donation, your child’s personalized birthday book will be included in our library’s exceptional collection for this year and many years to come.
