I, the parent/guardian of the above-named participant, understand that participation in basketball and related activities involves inherent risks, including but not limited to injury, illness, or accident. I voluntarily assume all risks associated with my child’s participation at Claymont Community Center. I hereby release, waive, and discharge Never Xcluded Hoops (NXH), its owner, staff, coaches, volunteers, and partners, including Claymont Community Center, from any and all liability, claims, demands, or causes of action arising out of or related to any injury or loss that may occur during participation. I certify that my child is physically able to participate in athletic activities and that I am responsible for any medical expenses that may arise. I grant permission for my child to participate fully in all league activities.
I also give permission for photographs and/or videos of my child to be used for promotional purposes by NXH.
I understand that all registration fees are non-refundable unless otherwise stated.
I understand that my child’s registration information may be shared with league partners, including Jr. WNBA program administrators and the Claymont Community Center, for the purpose of league operations, communication, and participation requirements. I agree to receive communication regarding league updates, schedules, and important announcements via email, text, or team communication platforms.
