    Vendor Registration & Commitment Form

    To ensure proper space and promotion for all our vendors, we ask that you sign our Vendor Commitment Form. By signing this form, you are confirming that you are attending the Fifth Annual 5Heart Earthworm Festival on Saturday, October 17, 2026 and will be set up by 9:30am and will stay until the event ends at 4:00pm to guarantee the return of your deposit. You agree to bring all necessary equipment and materials for your vendor site. If you cannot attend, you understand that you must give the festival coordinator at least 24-hour notice. Cancellations within 7 days of the event will not receive a refund of the registration fee.
