I hereby give this minor permission to wrestle in Dover Youth Wrestling’s Tournament and release all sponsoring bodies, including the Dover Area School District, Dover Youth Wrestling Club, tournament officials, committees, volunteers, and referees from liability of injuries and damage, including travel to/from. I certify that the above information is correct, and the participant and all attendees are covered by medical insurance. With this, we will attend and the contestant will participate at my own risk and free will.
