Wonder Boxed Community STEM Club Participation & Liability Waiver
I hereby grant permission for my child to participate in activities associated with the Wonder Boxed Community STEM Club, including hands-on STEM experiments, science and engineering challenges, creative projects, technology activities, literacy connections, group activities, and indoor and outdoor programming.
I understand that participation may involve certain inherent risks, including physical activity, hands-on materials and experiments, use of age-appropriate tools or equipment, and participation in group activities. In consideration of my child’s participation, I voluntarily assume the risks associated with these activities.
I hereby release, waive, discharge, and hold harmless Wonder Boxed STEM Education Inc., Wonder Boxed LLC, and their officers, employees, instructors, volunteers, agents, affiliates, and representatives from claims, liabilities, damages, or causes of action arising from or related to my child’s participation in the Community STEM Club, except where prohibited by law.
In the event of an emergency or injury, I authorize Community STEM Club staff to seek appropriate medical attention for my child if I cannot be reached. I understand that I am responsible for any medical expenses incurred.
By signing below, I acknowledge that I have read, understand, and agree to the terms of this Community STEM Club Participation & Liability Waiver.
