PERFORMANCE PROGRAM REGISTRATION FORM

The performance program trains 3 hours per week in acrobatics/tumbling, dance technique, and choreography. Select groups will perform at 1-2 exhibitions and/or competitions throughout the season. There will be 1 costume required. The Performance Program is eligible by invitation or audition ONLY.

The Performance Program classes will be held in 6 week sessions. All memebers join by invitation or audition only. All members commit to the program until May. All members must buy a costume. All members are able to take any other dance or gymnastics classes at the studio for an additional $10 per weekly class.

The perfermance program is $15 per class. You may pay $45 per week, $90 every 2 weeks, $135 every 3 weeks or $270 per session. You must set up automatic payments via, Venmo, Zelle or Paypal.

RELEASE: I understand dance & gymnastics are sport activities with risk of injury. I do hereby hold free from liability Flippin’ Fabulous Dance LLC., and its instructors.
