** The deadline for filling out this form AND delivering your artwork to the O'Hanlon gallery is Saturday, Aug 29 at 1PM. The days and times for artwork delivery to the gallery are: Friday, Aug 28, 11am to 4pm and Saturday, Aug 29, 10am to 1pm. Please note size/weight restrictions:Floor or pedestal sculptures must not be larger than 36” in any direction or weigh more than 20lb. Wall sculpture must not be larger than 40” wide or 60” tall or extend out from the wall more than 12” or weigh more than 10lb.
