Program Schedule & Daily Experience
Lake Toho Athletics provides a structured after-school experience from school dismissal until 6:15 PM.

Students participate in a rotating schedule that includes:

Daily Schedule
Dismissal – 3:30 PM
Check-in • Snack • Bathroom • Pod Groups

3:30 – 4:45 PM - First Rotation:

Group 1: Athletic Development & Soccer Training
Group 2: Study Hall & Learning Routines
4:45 – 5:00 PM: Transition & Reset

5:00 – 6:15 PM - Second Rotation:

Group 2: Athletic Development & Soccer Training
Group 1: Study Hall & Learning Routines
6:15 PM: Pickup

Lake Toho Athletics is committed to providing a safe and organized dismissal process. Please provide accurate pickup information for your child.

Authorized Pickup Persons

Please list all individuals who are authorized to pick up your child from Lake Toho Athletics.

Informed Consent, Assumption of Risk, and Acknowledgement
I hereby give permission for my child to participate in Lake Toho Athletics by OTF (“LTA”), including all activities, programming, and events associated with the program during the selected enrollment period.

Lake Toho Athletics provides an after-school enrichment experience that may include soccer technical development, athletic development, speed and agility training, movement activities, games, physical conditioning, classroom activities, study time, and other age-appropriate activities.

As a condition of my child's participation in Lake Toho Athletics, I acknowledge and agree to the following:

• I understand that participation in athletic and physical activities involves inherent risks, including but not limited to falls, collisions, contact with other participants or equipment, muscle strains, sprains, fractures, and other injuries that may occur during participation.
• I understand that while Lake Toho Athletics will take reasonable steps to provide a safe and supervised environment, injuries may occur as a result of participating in physical activities.
• I voluntarily assume all risks and hazards associated with my child's participation in Lake Toho Athletics, including activities occurring during training sessions, games, enrichment activities, transitions, and other program-related activities.
• I release, waive, discharge, and hold harmless Lake Toho Athletics by OTF, Optimal Training Foundation, its owners, officers, directors, coaches, employees, contractors, volunteers, representatives, affiliates, sponsors, and agents from any and all claims, demands, causes of action, damages, or liability arising from or related to my child's participation in the program, except where prohibited by law.
• I understand and agree that this release applies to injuries or damages arising from participation in program activities, travel to and from scheduled activities when applicable, and use of facilities or equipment associated with Lake Toho Athletics.
• In the event of an injury or medical emergency, I authorize Lake Toho Athletics staff to seek appropriate medical attention for my child if I cannot be reached immediately. I understand that I am responsible for any medical expenses incurred.
• I certify that my child is physically able to participate in Lake Toho Athletics activities and that I have disclosed any medical conditions, allergies, restrictions, or accommodations that may affect my child's participation.
• I agree to notify Lake Toho Athletics of any changes to my child's medical information, emergency contacts, or ability to safely participate.
• I understand that participation requires my child to follow program expectations, safety rules, coach instructions, and behavior standards established by Lake Toho Athletics.
  By clicking "I Agree", I acknowledge that I have read, understand, and voluntarily agree to the terms of this Informed Consent, Assumption of Risk, and Acknowledgement.

Confirmation

BY AGREEING ABOVE AND SUBMITTING THIS FORM, I AM DELIVERING AN ELECTRONIC SIGNATURE THAT IS EQUIVALENT TO AN ORIGINAL MANUAL PAPER SIGNATURE. THE ELECTRONIC SIGNATURE WILL BE EQUALLY AS BINDING AS AN ORIGINAL MANUAL PAPER SIGNATURE.

Membership Billing Authorization

Tuition is billed monthly through automatic recurring payments. Your selected membership will renew each month during the school-year program unless canceled according to Lake Toho Athletics policies.
