By signing below I understand that payment is due in full prior to my child’s participation. I agree to submit the full amount per child and acknowledge that fees are non-refundable except in cases determined by program administration.

I understand that participation in Homeschool Soccer Academy involves physical activity and carries inherent risks. By signing below, I confirm that my child is physically and mentally capable of performing physical tasks such as running, jumping, following instructions and other sport activities. I release Homeschool Soccer Academy, coaches, volunteers and program organizers from any liability for injuries, accidents, or damages that may occur during practices, games, or related activities.

I acknowledge that I am responsible for my child’s medical expenses arising from participation.

By signing below, I acknowledge that I have read and understand the NJ Sports Concussion and Head Injury Information and Acknowledgment form attached above. I also acknowledge that I have went over the information with my child and they understand the risks of participating in this program and are aware to inform their parent/guardian and coach immediately if they have any symptoms of a concussion or any type of injury.

In the event of an emergency where I cannot be reached, I authorize the coaches, program organizers, or designated volunteers to seek medical care for my child. I agree to assume all costs associated with such care.

By signing below I acknowledge that my child may be captured in photographs and videos. I grant Homeschool Soccer Academy permission to take photos or videos of my child during program activities for promotional purposes, including social media, flyers, and program updates.

Homeschool Soccer Academy (HSA) is committed to providing a welcoming, respectful and bully-free environment for all participants. Bullying, harassment, intimidation, threatening behavior, deliberate exclusion, or repeated inappropriate behavior toward another participant will not be tolerated.

HSA coaches and staff will make reasonable efforts to address inappropriate behavior that they personally observe or that is brought to their attention.

Parents and guardians are expected to promptly notify an HSA coach or the program owner if they witness bullying, harassment, unsafe behavior, or have concerns regarding their child's interactions with another participant. Whenever possible, concerns should be reported as soon as they occur so that HSA has an opportunity to investigate and address the situation promptly.

HSA staff cannot reasonably respond to behavior or concerns that they did not witness and that were not reported to them. A parent or guardian's failure to report a concern may prevent HSA from having the opportunity to investigate the circumstances or take appropriate corrective action at the time of the incident.

Depending on the circumstances, HSA may respond to inappropriate behavior through redirection, parent involvement, temporary removal from an activity, behavioral expectations, or removal from the program when necessary to protect the safety and well-being of participants.

By registering my child, I acknowledge that I have read and understand HSA's Bullying, Conduct & Parent Reporting Policy and agree to promptly communicate concerns to HSA so that the program has a reasonable opportunity to address them.

Nondiscrimination Policy:
Homeschool Soccer Academy admits participants of any race, color, national origin, sex, religion, and disability and does not discriminate on the basis of any legally protected status in the administration of its programs, activities, or policies.

Right to Refuse Service:
Homeschool Soccer Academy reserves the right to refuse service or remove any participant from the program at its sole discretion for reasons including, but not limited to: safety concerns, disruptive or inappropriate behavior, failure to comply with program rules, lack of payment, or any conduct that interferes with the safety, well-being, or integrity of the program. This right will be exercised in a fair and consistent manner and never on the basis of a protected characteristic.
