Parent/Guardian Agreement, Waiver, and Release of Liability

As the parent or legal guardian of the minor participant named above, I acknowledge that participation in theatrical, dance, movement, or other physical and performance-based activities at The West Side Show Room (WSSR) carries inherent risks, including but not limited to physical injury, illness, or property damage.

I voluntarily assume all such risks on behalf of my child and agree to release, indemnify, and hold harmless WSSR, its employees, contractors, board members, volunteers, and representatives from any and all liability, claims, demands, or causes of action that may arise from my child's participation in WSSR activities, whether caused by negligence or otherwise, to the fullest extent permitted by law.

I understand that this release includes any claims for the negligent acts or omissions of WSSR or its staff and covers bodily injury, illness, death, and property damage.

Medical Disclosure

I affirm that my child is physically able to participate in the activities offered and agree to inform WSSR in writing of any medical conditions, allergies, or accommodations needed before participation begins.

Photo and Media Release

I authorize WSSR to record, photograph, livestream, or otherwise capture my child's image during WSSR programs. I grant full rights to use these images or recordings in any format for publicity, archival, or promotional purposes, without compensation or further consent.

Additional Terms

This agreement is governed by the laws of the State of Illinois. Any legal disputes shall be resolved in the courts of Winnebago County, Illinois. If any part of this agreement is found to be unenforceable, the remainder shall remain in full effect.
