Informed Consent: By signing below, I hereby give my approval for my child’s participation in any and all activities prepared by Growing Chefs! Ontario during the selected program. In exchange for the acceptance of said child’s candidacy by Growing Chefs! Ontario, I assume all risk and hazards incidental to the conduct of the activities, and release, absolve and hold harmless Growing Chefs! Ontario and all its respective officers, agents, and representatives from any and all liability for injuries to said child arising out of, traveling to, participating in, or returning from selected program sessions. In case of injury to said child, I hereby waive all claims against Growing Chefs! Ontario including all program staff and participants, and, if applicable, owners and lessors of premises used to conduct the program.
Medical Release and Authorization: By signing below, the parent and/or legal guardian of the named participant, hereby consent to and authorize any qualified and licensed physician or other regulated health professional to provide necessary emergency medical care to the minor where, in the opinion of the attending professional, such care is required to prevent further endangerment of the minor’s life, physical disfigurement, physical impairment, or other undue pain, suffering or discomfort if they cannot be reached in a timely manner. This consent includes authorization for examinations, diagnostic procedures (including x-rays), and necessary medical treatment, including minor surgical procedures, as considered appropriate under the circumstances. In the event of serious illness, major surgery, or significant injury, reasonable efforts will be made to contact me prior to treatment; however, treatment may proceed without prior consent if I cannot be reached and delay would pose a major risk to the minor’s health or safety.I further authorize Growing Chefs! Ontario and its staff to provide or arrange for initial emergency care and transportation to an appropriate medical facility, as required. This authorization is valid for the duration of the participant’s involvement in the registered program and is provided voluntarily for the purpose of safeguarding the minor’s health and well-being in my absence.
Cancellation and Refund Policy:

Cancellations must be communicated by email to Education Programs Manager christy@growingchefsontario.ca as early as possible. Cancellations received more than 14 days before the first class will receive a full refund minus an administrative fee of $20 per participant. Cancellations received less than 14 days before the first class, provided we are successful in refilling their spot, are eligible to receive a full refund minus an administrative fee of $20 per participant. If we are unsuccessful, despite efforts in filling their spot, a partial refund of 50% will be offered. Cancellations received within 3 days of the start of the program, are ineligible to receive a refund, except in extenuating circumstances as approved by the Education Programs Manager. We are unable to offer credits to future classes, or to transfer the spot to another participant of your choosing (unless it is a sibling within the correct age range). Refunds are not granted for missed days, or if a parent/guardian withdraws the participant from the program or if the participant is sent home for misconduct.
