This is registration for CSD KODA and Deaf Camp Fall Connect Day.
We welcome all KODA, Deaf, or Hard of Hearing families to join us:
Candlebrook Farms

Sunday October 4th from 4pm - 8pm.
Entry fee is $5 per attendee. Payment will be collected at the end of this form. This event will be limited to 100 attendees.
Attendees must have registered and paid the event entry fee to enter.
Each registered attendee will get access to Candlebrook Farms Fall Festival: food buffett, hayrides, corn mazes, peddle bikes, fire pits, children's play area, sunflower patch, smores, and a small individual pumpkin to decorate.
A full agenda of the evening will be shared closer to the event date with those who have registered.
Their Fall Shop and U-Pick Pumpkin Field will remain open until 7:30pm. The cost of these are not included in this event.
Interpreters will be on site as well as our staff to answer any questions you may have.
