I, as the parent/guardian of the participant, hereby declare that my son/daughter is in good health and physically fit to participate in this competition. I agree and authorize the organizer & co-organizer to take photographs and videos during the competition for promotional purposes, including usage on official websites and social media platforms. I and my child have read, fully understood, and agree to abide by all the competition rules, terms, and conditions.

本人為參加者的家長/監護人，特此聲明本人兒子/女兒身體健康狀況良好，適宜參加是次比賽。本人同意主辦及協辦單位於比賽期間進行拍攝及錄影，並授權該宣傳素材用於官方網頁、社群媒體及相關宣傳用途。本人及本人子女已詳細閱讀所有比賽規則、條款及細則，並清楚明白及承諾完全遵守。
