WAIVER & LIABILITY RELEASE
I am the parent or legal guardian of {childsInformation} and give permission for my child to participate in the COLORS Yoga for Kids Yoga + Craft Workshop, including yoga, movement activities, games, relaxation exercises, and the creation of a glitter jar using craft materials.
I understand and acknowledge the following:
Assumption of Risk: I am aware that participation in physical activities such as yoga involves certain inherent risks, including but not limited to physical injury. I knowingly and voluntarily assume all such risks.
Drop-Off and Pickup Policy: The building doors will remain locked during the workshop. Children will only be released to the adult who dropped them off or an adult listed as authorized for pickup. If pickup plans change, the parent or guardian who completed drop-off must call COLORS Yoga before pickup to authorize the change. Identification may be requested.
Behavioral Expectations: I understand that I may be required to pick up my child early if, after reasonable redirection, their behavior remains unsafe, intentionally destructive, or poses a risk of harm to themselves, others, or property. No refund will be provided in such cases.
Photo/Video Release: I grant permission for my child's photograph or video to be taken during the event for promotional or educational purposes related to the yoga program.
Medical Authorization: In the event of an injury or medical emergency involving my child, I authorize COLORS Yoga for Kids to seek medical treatment for my child if I am not immediately available or unable to consent for any reason.
Waiver and Release: I, on behalf of myself, my child, heirs, executors, administrators, and assigns, hereby waive, release, and discharge COLORS Yoga for Kids, its officers, employees, and agents from any and all claims, liabilities, demands, actions, and causes of action whatsoever, arising out of or related to any loss, damage, or injury, including death, that may be sustained by my child or to any property belonging to my child, whether caused by the negligence of COLORS Yoga for Kids or otherwise, while participating in the kids yoga class.
I have read this waiver and release of liability, fully understand its terms, understand that I have given up substantial rights by signing it, and sign it freely and voluntarily without any inducement.
