Creative Roots Studio Art Registration Form-Fall 2026

Attendance & Make-Up Policy 

Because class sizes are intentionally small, regular attendance is encouraged. If your child misses a scheduled class, one make-up class may be scheduled each calendar month, based on availability and with advance notice. Make-up classes cannot be carried over to future months and have no cash value.

Drop-In Class Policy

Creative Roots Studio welcomes drop-in students when space is available.

Drop-in classes are $25 per class and must be paid in full before the class begins.
Advance registration is required, as class sizes are limited to 6 students.
Drop-in students participate in the scheduled lesson for that day.
If a registered drop-in student needs to cancel, at least 24 hours' notice is required to receive a credit toward a future drop-in class. Cancellations made with less than 24 hours' notice and no-shows are non-refundable.
Students who enjoy the class are welcome to enroll in the semester program at any time, pending space availability.

Registration Acknowledgment & Payment Agreement

By signing this registration form, I acknowledge that I am enrolling my child in Creative Roots Studio for the selected semester. I understand that tuition may be paid monthly or in full for the semester.

If I choose the monthly payment option, I understand that an invoice will be sent each month throughout the semester, and payment is due upon receipt (or by the due date listed on the invoice). If I choose the semester payment option, the full tuition is due before the first class unless other arrangements have been approved.

I understand that Creative Roots Studio reserves my child's space for the entire semester. Missed classes may be made up once per month, subject to availability and with advance notice. Makeup classes do not carry over beyond the current month and are not refundable.

I have read and agree to the Creative Roots Studio registration, payment, and attendance policies.

Sickness Policy

The health and well-being of our students and families is important to us. To help maintain a healthy learning environment, please keep your child home if they:

Have a fever of 100.4°F or higher within the past 24 hours.
Have vomited or had diarrhea within the past 24 hours.
Have a contagious illness (such as strep throat, pink eye, chickenpox, COVID-19, influenza, etc.) and have not been cleared to return.
Have excessive coughing, difficulty breathing, or other symptoms that may interfere with participation or pose a risk to others.
Children should be fever-free for at least 24 hours without the use of fever-reducing medication before returning to class.

If your child becomes ill during class, the parent or emergency contact will be notified and asked to pick up the child promptly.

Students who miss class due to illness may schedule a make-up class once per month, subject to availability and advance notice.

Weather & Emergency Closure Policy

Creative Roots Studio follows the Omaha Public Schools (OPS) weather-related closure and delay decisions whenever possible.

If Omaha Public Schools closes due to inclement weather, Creative Roots Studio will also be closed.
If Omaha Public Schools announces a late start, early dismissal, or cancellation of after-school activities, Creative Roots Studio reserves the right to adjust or cancel classes as needed.
Families will be notified as soon as possible by email and/or text if a class is canceled due to weather or another emergency.
Classes canceled by Creative Roots Studio because of weather or emergency conditions will be rescheduled whenever possible. If a make-up class cannot be scheduled, a credit will be applied toward a future class.
The safety of our students, families, and staff is always our highest priority.
