**Disclosure** Conduct of Model and Guests* This field is required.Conduct is extremely important to the success of ever Runway Modeling Experience Production. Models, as well as Friends and/or Family supporting the model in this production is expected to conduct themselves professionally and appropriately. If a Model and/or friend or family member is found to perform actions of misconduct, in-person, over the phone, thru text messages, over any social media platforms and/or thru email, the model will be IMMEDIATELY removed from production and all forms of communication for the production and permanently banned from any future endeavors with Nita Belles Closet on a PERMANENT bases!

Signature* This field is required.Model Payment Agreement for Showcase Participation By registering for and participating in the showcase, you acknowledge that you have read, understood, and agree to the following terms and conditions: 1. You agree to follow the payment arrangement established for your participation in the showcase. All payments must be received on or before their scheduled due dates. 2. Any payment not received by its due date will incur a $25.00 late fee for each late payment. 3. If any payment remains unpaid for more than three (3) calendar days after the due date, you will be removed from the showcase registration. 4. Reinstatement after removal is not guaranteed and may only be approved at the sole discretion of the designer. If reinstatement is approved, all outstanding balances and applicable late fees must be paid before participation will be restored. 5. By electronically accepting this agreement, you acknowledge that this electronic acceptance has the same legal effect as a handwritten signature and that you agree to comply with all terms outlined in this agreement as a condition of participating in the showcase.
