EDS SUMMER FREESTYLE BATTLE - LIABILITY WAIVER & MEDIA RELEASE
1. Assumption of Risk: I acknowledge that participation in the EDS Summer Freestyle Battle involves physical exertion, dance movements, and inherent risks of physical injury. I voluntarily assume all risks associated with participation, including but not limited to bodily injury, illness, or property loss.
2. Release of Liability: I hereby release, waive, and forever discharge Enjoy Dance Studio ("EDS"), its owners, instructors, staff, and event organizers from any and all claims, demands, liabilities, or causes of action arising out of or related to any loss, damage, or injury that may be sustained by me during the event.
3. Media & Photo Release: I grant EDS full permission to capture, use, publish, and distribute photos, videos, and audio recordings of my performance during the event for promotional, marketing, and social media purposes without financial compensation.
4. Medical Authorization: In the event of an emergency, I authorize EDS staff to obtain necessary emergency medical treatment on my behalf (or on behalf of my minor child). I understand that I am solely responsible for any medical costs incurred.
5. Confirmation of Age/Authority: If signing on behalf of a minor (under 18 years of age), I confirm that I am the parent or legally authorized guardian with full authority to sign this waiver.
