A proven architect of elite development, Gary recently served as Director of Coaching for Real Salt Lake City’s U18 & U16 Academies, while leading the Elite Development Program for the U15–U12 pathway (2024–26). His influence stretches far beyond one club: from guiding National ODP teams to shaping soccer cultures as a USSF Technical Director for 26 years in Texas and Oklahoma, he’s helped carve pathways for players now honored in the National Soccer Hall of Fame.

Informed Consent and Acknowledgement

I hereby give my approval for my child’s participation in any and all activities prepared by xG Factor or xG Factor Coaches during the selected camp. In exchange for the acceptance of said child’s candidacy by xG Factor, I assume all risk and hazards incidental to the conduct of the activities, and release, absolve and hold harmless xG Factor or xG Factor Coaches. and all its respective officers, agents, and representatives from any and all liability for injuries to said child arising out of traveling to, participating in, or returning from selected camp sessions.

In case of injury to said child, I hereby waive all claims against xG Factor, including all coaches and affiliates, all participants, sponsoring agencies, advertisers, and, if applicable, owners and lessors of premises used to conduct the event. There is a risk of being injured that is inherent in all sports activities, including soccer. Some of these injuries include, but are not limited to, the risk of fractures, paralysis, or death.

Medical Release and Authorization

I, as the parent/legal guardian of the child/children listed above authorize said child’s participation in xG Factor event. In and for consideration of my child’s participation in the event, I agree that I will not hold xG Factor, staff, Lake Dallas, or its employees responsible for any loss, damages, or personal injuries that my child may receive as a result of participation. This waiver of liability expressly includes event and camp activities, or while in, on or upon the premises whereby the activity is being conducted and transportation to and from, or in connection with said event or camp. I also agree that my child will follow all instructions and procedures in order to maintain a maximum level of safety. I also give my permission for any emergency medical care or treatment by a trainer or coach.

I understand the above mentioned conditions, and my child/children has/have my permission to participate in the activities planned for the xG Factor Camp.

BY ACKNOWLEDGING AND SIGNING BELOW, I AM DELIVERING AN ELECTRONIC SIGNATURE THAT WILL HAVE THE SAME EFFECT AS AN ORIGINAL MANUAL PAPER SIGNATURE. THE ELECTRONIC SIGNATURE WILL BE EQUALLY AS BINDING AS AN ORIGINAL MANUAL PAPER SIGNATURE.
