Important Notice to Exhibitors

Cat Details:

• All details must be entered exactly as shown on the pedigree, including the cat’s registered name, abbreviated titles, and colour.
• Please ensure all information entered is correct. Typographical errors or incorrect class selections may result in incorrect paperwork and the exhibit being entered in the wrong class.
• Please complete the form in the order the questions are presented. Do not skip or jump ahead, as this may result in missing or incomplete information being submitted.
• Non-FCCV members must upload a copy of the pedigree at the time of application.
• After submission, you will receive an automatic confirmation email containing the information you entered. If corrections are required, please follow the instructions in that email to create a Jotform account using the same email address you have entered for the submission. Editing will not be permitted after the entry has closed.
• Please inform the show manager on any title upgrades by Monday 5:00pm on the show week.
• The Show Manager reserves the right to refuse any entries at their discretion.

Each cat must be entered using a separate submission.
If you are entering multiple cats, please submit the form separately for each cat. You will have the option to make a combined payment for multiple entries.

Payment:

• Payment must be completed before entries close. If payment is not received by the closing date, the Show Manager reserves the right to reject the entry. 
• WE ACCEPT: Bank Transfer and Credit Card (including Apple Pay).
• If paying online, please select the quantity (number of cats) you are entering. The total price will automatically adjust based on the number selected.
• To purchase a catalogue, please tick the checkbox on the left before payment.
• Online payment is processed via Square. If you pay by credit card, Jotform (the host of this form) will send a separate email containing a link to your Square payment receipt.

Entry Fee:

First 3 cats: $60.00
4th cat onwards: $45.00
Pack Up Fee per exhibitor (compulsory): $6.00
Catalogue: $10.00

We encourage exhibitors to pay by bank deposit where possible, as the Association incurs a surcharge when accepting card payments.

1 cat: $60; 2 cats $120; 3 cats $180; 4 cats $225; 5 cats $270;
6 cats $315; 7 cats: $360; 8 cats $410; 9 cats $405; 10 cats $450.

By submitting the entry:

I the undersigned, hereby certify that by entering this cat in the show, I have agreed to the following terms and conditions:

• The above exhibit is owned or leased by me and is registered with the FCCV Inc. or an approved Governing Body.
• The exhibit is eligible for the classes entered.
• I enter the exhibit at my own risk.
• I agree to observe and be bound by the regulations and decisions of the show and of the FCCV Inc. Committee and accept their decision as final.
• I am a financial member of the FCCV Inc. or the Governing Body as stated by me on this entry form.
• I have read the Regulations regarding Veterinary Services on the FCCV Inc. website By-law 4.4.
• I agree that all other CCCA member bodies will be notified if this cat is judged UTH.
• To the best of my knowledge and belief my exhibit has not been exposed to the risk of any infectious or contagious disease since the date which is fourteen (14) days before this Show.
• I will not exhibit the exhibit at this Show if on or between now and the day of the Show they incur such risk, or otherwise become ineligible.
