Participant Agreement, Liability Release, Medical Authorization & Media Release
This registration must be completed by a parent or legal guardian for participants under the age of 18. Participants who are 18 years of age or older may complete and sign this registration on their own behalf.

By signing this form electronically, I acknowledge that my signature is legally binding and that I have read and agree to the terms outlined below.

I understand that participation in Called Deeper, hosted by West Florida Ministry Network Student Ministries, involves activities that carry inherent risks, including but not limited to transportation, recreational activities, physical activity, and participation in group events. While every reasonable precaution is taken to provide a safe environment, accidents and injuries can occur.

On behalf of myself and/or my child, I voluntarily assume all risks associated with participation in this event.

To the fullest extent permitted by Florida law, I release and hold harmless the West Florida Ministry Network Assemblies of God, West Florida Student Ministries, the event host facility, participating churches, church leaders, volunteers, employees, directors, officers, agents, and representatives from any and all claims, liabilities, damages, losses, expenses, or causes of action arising from participation in this event, except where prohibited by law.

I understand that participants are expected to follow all event rules and instructions provided by event staff and church leadership. Failure to do so may result in dismissal from the event without refund.

This agreement shall be governed by the laws of the State of Florida.

Medical Authorization

I certify that the participant is physically able to participate in the activities associated with Called Deeper unless otherwise disclosed on this registration.

In the event of illness or injury, I authorize West Florida Student Ministries personnel, host church representatives, or designated adult leaders to obtain emergency medical treatment for the participant if I cannot be reached immediately.

I understand that every reasonable effort will be made to contact the parent, legal guardian, or emergency contact before treatment whenever possible.

I accept responsibility for any medical expenses incurred as a result of treatment.

Supervision

I understand that all students remain under the supervision and responsibility of their attending church's designated leaders and/or their parent or legal guardian throughout the event.

West Florida Student Ministries provides event leadership but does not assume custodial responsibility for participants.

Photo & Media Release

I grant permission to the West Florida Ministry Network Assemblies of God and West Florida Student Ministries to photograph, record, or video the participant during Called Deeper.

I understand that these photographs, videos, audio recordings, and other media may be used without compensation for lawful ministry purposes, including:

• Promotional materials
  Social media
  Websites
  Printed publications
  Future event promotion
  Ministry presentations

I understand that images or recordings may be edited and reproduced without further approval.

I waive any right to inspect or approve the finished product and waive any claim for compensation arising from the use of these materials.

This authorization remains in effect unless revoked in writing prior to the event.

Parent/Guardian or Adult Participant Certification
By signing below, I certify that:
